Cardiovascular disease (CVD) is an overarching term that describes a family of diseases (cardiac and vascular, stroke, renal, and diabetes), each sharing a common set of risk factors.

Overarching aim

To strengthen the outcomes for CVD patients in London. This will be achieved through a whole systems, integrated approach involving expert clinical leadership, design and designation of care pathways, the support of stronger commissioning and the measurement of quality markers against defined standards.
